We are seeking a dedicated and experienced Nurse Practitioner to provide clinical leadership, comprehensive patient care, and support for our home visit program in Manhattan. Overview

As a key clinical leader, you will promote compliance with quality and cost standards, managing service delivery for individual cases and populations. You will serve as a vital resource, offering clinical and operational guidance to team members, and delivering advanced nursing care in accordance with state, federal, and national standards. This position reports directly to the Clinical Director for the Nurse Practitioner Program.

What You Will Do 🔧

• Provide full-scope advanced nursing practice for targeted patient populations, evaluating therapy responses and revising care plans as needed to ensure quality outcomes
• Identify the need for new or revised clinical protocols; collaborate with physicians and team members to develop and train on these protocols
• Deliver comprehensive care including physical examinations, health screenings, emergent interventions, pharmacological and non-pharmacological treatments, health maintenance, care management, referrals, discharge planning, counseling, and patient education
• Establish treatment plans based on clinical findings, determining when further evaluation is necessary by physicians or specialists
• Work closely with patients, families, primary care providers, and team members for assessment, care planning, and ongoing management across various settings
• Ensure clinical services meet Patient-Centered Medical Home standards, meaningful use of medical records, and quality measurements such as HEDIS and QARR

Qualifications ✨

Licenses and Certifications ✅

• Current registered nurse license and registration in New York State
• Active Nurse Practitioner certification in adult health, family health, or gerontology in New York State
• Valid driver’s license (regional needs-dependent)
• Credentialed status with VNS Health Medical Care at Home and managed care plans
• Possession of NPI, Medicaid, and Medicare provider numbers preferred
• Collaboration agreement with a physician (NY State regulation - preferred)
• Board certification by ANCC or equivalent (required for billing Medicare)
• **For Psychiatric Nurse Practitioners:** PMHNP-BC certification required

Education 🎓

• Master’s Degree in Nursing or a graduate degree from an approved NP program
• Current ANCC or AANP certification in adult, family, or geriatric nurse practitioner roles
• **For Psychiatric Nurse Practitioners:** Master's in psychiatric-mental health NP; PhD preferred

Work Experience 💼

• Minimum of two years practicing as a nurse practitioner, utilizing full scope of practice (preferred)
• Experience in clinical home care or management roles (preferred)
• Knowledge of HEDIS and QARR quality measures, ICD-10, CPT coding for reimbursements
• Bilingual skills, based on operational needs

Compensation 💵

Annual salary ranges from USD $109,900.00 to USD $146,500.00.
