Overview

Provides clinical leadership to promote increased compliance with quality and cost standards of care. Manages the service delivery of inter-professional and para-professional teams, acting as a key resource in delivering clinical and operational support. Delivers advanced nursing clinical care aligned with State and Federal regulations within the Nurse Practitioner's scope of practice, under the supervision of the Clinical Director for the Nurse Practitioner Program. 🌍🩺

What You Will Do

• Manage and provide full scope of advanced nursing practice for targeted patient populations. Evaluate responses to therapy and adjust plans to achieve quality outcomes.
• Identify need for new or revised clinical protocols, collaborate with physicians to develop training and implementation.
• Deliver comprehensive nursing care including physical exams, history taking, screening, emergent interventions, treatment ordering, preventative care, case management, referrals, discharge planning, counseling, and education. Establish treatment plans based on clinical findings and determine need for further evaluation.
• Collaborate with patients, families, primary care physicians, and team members to assess, plan, and coordinate ongoing care management across various settings.
• Ensure clinical services compliance with Patient-Centered Medical Home standards, meaningful use of medical records, HEDIS, and QARR quality measures.

Qualifications

Licenses and Certifications

• 📝 Current Registered Professional Nurse license and registration in New York State
• 📝 Current Nurse Practitioner certification in adult health, family health, or gerontology in New York
• 🔑 Valid driver’s license (as needed)
• ✔️ Credentialed status with VNS Health Medical Care at Home and associated plans
• 📇 Maintains NPI, Medicaid, and Medicare provider numbers (preferred)
• 🤝 Collaborative practice agreement with a physician (preferred)
• ✔️ Certification by ANCC or equivalent (required for billing Medicare)
• 🔹 For Psychiatric Nurse Practitioners: Current PMHNP-BC certification required

Education

• 🎓 Master’s Degree in Nursing or equivalent from a registered nurse practitioner program approved by the NYS Education Department
• ✔️ Current ANCC or AANP certification as an adult, family, or geriatric nurse practitioner
• 🔹 For Psychiatric Nurse Practitioners: Master’s in psychiatric-mental health Nurse Practitioner; PhD preferred

Work Experience

• 🕒 Minimum of two years’ experience as a nurse practitioner utilizing full scope of practice (preferred)
• 🏥 Clinical home care or managerial experience (preferred)
• 📊 Knowledge of HEDIS, QARR measures, ICD-10, CPT coding (required)
• 🗣️ Bilingual skills based on operational needs (required)

Pay Range

USD $109,900.00 - USD $146,500.00 /Yr.
