Overview

As a key clinical leader, you will promote compliance with quality and cost standards of care, manage service delivery alongside inter-professional teams, and serve as a vital resource offering clinical and operational support. You will deliver advanced nursing care aligned with State, Federal, and national standards, working under the supervision of the Clinical Director for the Nurse Practitioner Program.

What You Will Do

• Manage and provide the full scope of advanced nursing practice for targeted patient populations.
• Evaluate patient responses to therapies and interventions, updating care plans to optimize outcomes.
• Identify the need for new or revised clinical protocols, collaborating with physicians and team members.
• Deliver comprehensive nursing care including:
  • Physical examinations and comprehensive histories
  • Screening for physical and psychological conditions
  • Emergent interventions and medication management
  • Preventative health activities and health maintenance
  • Care management, referrals, and discharge planning
  • Patient counseling and education
• Establish and adjust treatment plans based on clinical findings, determining when specialist or emergency evaluations are necessary.
• Collaborate with patients, families, primary care physicians, and team members for assessment and care planning.
• Ensure compliance with Patient-Centered Medical Home standards, meaningful use of medical record data, and quality measures like HEDIS and QARR.

Qualifications

Licenses and Certifications

• Active Registered Nurse license and registration in New York State
• Current Nurse Practitioner certification in adult health, family health, or gerontology
• Valid driver’s license (operational needs dependent)
• Credentialed with VNS Health Medical Care at Home and associated plans
• NPI, Medicaid, and Medicare provider numbers (preferred)
• Collaborative practice agreement with a physician (preferred)
• Certification by ANCC or equivalent body (required for billing Medicare)
• For Psychiatric Nurse Practitioners: PMHNP-BC certification (required)

Educational Requirements

• Master’s Degree in Nursing or higher from an accredited nurse practitioner program (required)
• Current ANCC or AANP certification as an adult, family, or geriatric NP (required)
• Master's in psychiatric-mental health Nurse Practitioner (required)
• PhD in psychiatric-mental health (preferred)

Work Experience

• Minimum two years as a nurse practitioner with full scope practice (preferred)
• Experience in home care or management (preferred)
• Knowledge of HEDIS, QARR, ICD-10, and CPT coding for reimbursement (required)
• Bilingual skills based on operational needs (required)

Compensation & Benefits

Salary Range: USD $109,900.00 – $146,500.00 per year 💰
