Overview

Provides clinical leadership to enhance compliance with quality and cost standards of care. Manages service delivery of inter-professional and para-professional teams, acting as a key resource for clinical and operational guidance. Delivers advanced nursing care in alignment with State and Federal regulations and national standards, operating under the supervision of the Clinical Director for the Nurse Practitioner Program.

What You Will Do

• Manage and deliver full scope of advanced nursing practices for targeted patient populations. Evaluate responses to therapies and revise care plans accordingly.
• Identify the need for new or revised clinical protocols; collaborate with physicians to develop and train staff on these protocols.
• Provide comprehensive advanced nursing care, including physical examinations, history taking, screening, interventions, treatments, referrals, discharge planning, counseling, and patient education. Formulate treatment plans based on clinical findings and refer for further evaluation when needed.
• Collaborate with patients, families, primary care physicians, and team members to assess needs and develop care plans, ensuring continuous management across settings.
• Ensure clinical services align with Patient-Centered Medical Home standards, meaningful medical record use, HEDIS, and QARR quality measures.

Qualifications

Licenses and Certifications

• Current Registered Nurse license and registration in New York State
• Current Nurse Practitioner license with specialty in adult health, family health, or gerontology (New York State)
• Valid driver’s license (may be required based on regional needs)
• Credentialed status with VNS Health Medical Care at Home and related managed care plans
• NPI, Medicaid, and Medicare provider numbers (preferred)
• Collaborative practice agreement with a physician (preferred)
• Certification by ANCC or another accredited Nurse Practitioner body (required for billing Medicare and credentialing)
• 🌟 For Psychiatric Nurse Practitioners: Current PMHNP-BC certification required

Education

• Master’s Degree of Science in Nursing or equivalent from an approved NP program (required)
• Current ANCC or AANP certification as adult, family, or geriatric nurse practitioner (required)
• 🌟 Psychiatric Nurse Practitioners: Master’s Degree in psychiatric-mental health Nurse Practitioner (required) and PhD preferred

Work Experience

• Minimum two years as a nurse practitioner utilizing full scope of practice (preferred)
• Clinical home care or managerial experience (preferred)
• Knowledge of HEDIS and QARR quality measures, ICD-10, and CPT coding (required)
• Bilingual skills based on operational needs (required)

Pay Range

USD $109,900.00 - USD $146,500.00 per year
