We are seeking a dedicated and experienced Nurse Practitioner to provide expert clinical leadership, ensuring quality care, compliance, and exceptional patient outcomes across our home health services in Manhattan.

Overview 🩺

The Nurse Practitioner will:

  • Provide clinical leadership to enhance compliance with quality and cost standards.
  • Manage service delivery by inter-professional and para-professional teams, handling individual cases or patient populations.
  • Offer clinical and operational guidance to teams to optimize team performance and patient care.
  • Deliver advanced nursing clinical care aligned with State and Federal regulations, national standards, and the Nurse Practitioner's scope of practice.
  • Work under the supervision of the Clinical Director of the Nurse Practitioner Program.
  • Conduct home visits within Manhattan.

What You Will Do 🏥

  • Manage and provide a full scope of advanced nursing practice for targeted patient populations.
  • Evaluate patient responses to therapies and modify care plans to ensure quality outcomes.
  • Identify the need for new or revised clinical protocols and collaborate with physicians to develop and train staff on these protocols.
  • Perform comprehensive clinical assessments, including physical exams, health histories, and screenings.
  • Implement emergent interventions, pharmacological and non-pharmacological treatments, and preventative health activities.
  • Coordinate care management, referrals, discharge planning, counseling, and patient education.
  • Establish and modify treatment plans based on clinical findings, consulting with physicians or specialists as needed.
  • Collaborate with patients, families, primary care providers, and team members for assessment and care planning.
  • Provide clinical services compliant with Patient-Centered Medical Home standards, meaningful use, and quality measures like HEDIS and QARR.

Qualifications & Certifications 📜

Licenses & Certifications ✅

  • Valid RN license in New York State
  • NY State Nurse Practitioner certification with a specialty in adult health, family health, or gerontology
  • Possession of a valid driver’s license (as required)
  • Credentialing with VNS Health Medical Care at Home and managed care plans
  • Maintaining NPI, Medicaid, and Medicare provider numbers preferred
  • Collaborative practice agreement with a physician (preferred)
  • Certification by ANCC or equivalent accrediting body — essential for billing Medicare

Specialized Certification for Psychiatric Nurse Practitioners 🧠

  • Current PMHNP-BC certification required

Educational Background 🎓

  • Master's Degree of Science in Nursing or a related graduate degree from an accredited NP program
  • Current ANCC or AANP certification as an adult, family, or geriatric nurse practitioner
  • Master’s in psychiatric-mental health Nurse Practitioner (required)
  • PhD in psychiatric-mental health Nurse Practitioner (preferred)

Work Experience 💼

  • Minimum of 2 years experience as a nurse practitioner (preferred)
  • Clinical home care or managerial experience (preferred)
  • Knowledge of HEDIS, QARR, ICD-10, and CPT coding for reimbursement (required)
  • Bilingual skills based on operational needs (required)

Compensation 💰

Annual Salary Range: USD $109,900 – USD $146,500
