We are seeking a dedicated Nurse Practitioner to provide expert clinical leadership, delivering comprehensive care to diverse patient populations in Manhattan, and fostering quality, compliance, and team collaboration in a supportive environment.

Overview

As a key clinical leader, you will promote compliance with quality and cost standards of care, managing service delivery among inter-professional teams on individual cases or populations. You will serve as a vital resource, offering clinical and operational guidance to ensure optimal team outcomes. This role involves delivering advanced nursing care in accordance with state, federal, and national standards of practice, working under the supervision of the Clinical Director for the Nurse Practitioner Program.

What You Will Do 💼

• Provide full scope of advanced nursing practice for targeted patient populations, evaluating responses to therapy and revising care plans to achieve quality outcomes.
• Develop and revise clinical protocols by collaborating with physicians and other team members, providing training as needed.
• Deliver comprehensive nursing care including assessments, screenings, emergent interventions, medication management, health maintenance, care management, referrals, discharge planning, counseling, and patient education. Establish and adjust treatment plans based on clinical findings.
• Collaborate with patients, families, primary care physicians, and team members to provide holistic assessment, care planning, and continuous management across settings.
• Ensure compliance with Patient-Centered Medical Home standards, meaningful use of medical records, and quality measures such as HEDIS and QARR.

Qualifications 🎓

Licenses and Certifications ✅

• Current RN license and registration in New York State
• NY State Nurse Practitioner certification in adult, family, or gerontology health
• Valid driver’s license (may be required based on operational needs)
• Credentialed status with VNS Health Medical Care at Home and related plans
• Maintains NPI, Medicaid, and Medicare provider numbers (preferred)
• Collaborative practice agreement with a physician (preferred)
• Certification by ANCC or equivalent (required for billing Medicare)
• For Psychiatric Nurse Practitioners: PMHNP-BC certification required

Educational Requirements 📚

• Master’s Degree in Nursing or related graduate program registered by NY State Education Department
• ANCC or AANP certification as an adult, family, or geriatric nurse practitioner
• For Psychiatric Nurse Practitioners: Master's in psychiatric-mental health nursing (required); PhD preferred

Experience & Skills 🌟

• Minimum two years of full-scope nurse practitioner experience (preferred)
• Home care or management experience (preferred)
• Knowledge of HEDIS, QARR measures, ICD-10, and CPT coding for reimbursement
• Bilingual skills (required based on operational needs)

Compensation 💰

Salary Range: USD $109,900.00 – USD $146,500.00 annually
