Overview
As an Advanced Practice Clinician, you will lead efforts to improve compliance with quality and cost standards of care. You will oversee service delivery by inter-professional and para-professional teams, offering clinical and operational guidance to optimize team outcomes. Working under the supervision of the Clinical Director for the Nurse Practitioner Program, you will deliver advanced nursing care aligned with regulatory and national standards.

What You Will Do 🩺
- Manage and deliver full-scope advanced nursing care for targeted patient populations, evaluating responses to therapy and revising care plans to ensure quality outcomes.
- Identify the need for new or revised clinical protocols; collaborate with physicians and team members to develop and train staff on these protocols.
- Provide comprehensive care including physical examinations, history-taking, screening, emergent interventions, medication management, preventative activities, referrals, discharge planning, counseling, and patient education.
- Develop treatment plans based on clinical findings and work with patients, families, and healthcare teams to coordinate ongoing assessment and care.
- Ensure clinical services meet standards such as the Patient-Centered Medical Home, meaningful use of electronic records, HEDIS, and QARR metrics.
Qualifications & Certifications ✅
Licenses and Certifications
- Current license and registration as a Registered Professional Nurse in New York State
- Certified Nurse Practitioner license in New York with a specialty in adult health, family health, or gerontology
- Valid driver’s license (may be required based on operational needs)
- Credentialed status with VNS Health Medical Care at Home & managed care plans
- Possession of NPI, Medicaid, and Medicare provider numbers (preferred)
- Collaborative practice agreement with a physician (preferred)
- Certification by ANCC or equivalent accrediting body — necessary for billing Medicare and credentialing
Specialized Certifications for Psychiatric Nurse Practitioners Only 🧠
- PMHNP-BC certification required
Educational Requirements 🎓
- Master’s Degree of Science in Nursing or related graduate degree from a recognized nurse practitioner program
- Current certification by ANCC or AANP as an adult, family, or geriatric nurse practitioner
- For Psychiatric Nurse Practitioners: Master's in psychiatric-mental health Nurse Practitioner; PhD in psychiatric-mental health (preferred)
Experience 🌟
- Minimum of two years of full-scope nurse practitioner experience (preferred)
- Clinical home care or managerial experience (preferred)
