More About the Opportunity

As an Activity Coordinator (PACE), you will be an integral part of our PACE-Clinic team, collaborating with providers, community partners, and local stakeholders to ensure top-quality, respectful care for our members. This position is approved for full office work, with potential telework options.

Note: If telework is approved, you must work within California. For partial telework, you are required to be at the Main Office in Orange, CA, at least two days per week.

Key Responsibilities

  • Develop, coordinate, and plan individual and group recreational activities for participants in Cal Optima Health's PACE.
  • Monitor and revise activities to meet participants' and program goals, ensuring regulatory compliance.
  • Foster a mission-driven, member-focused culture emphasizing customer service, dignity, consistency, and accountability.
  • Support department responsibilities and collaborate with team members to achieve both short- and long-term goals.
  • Coordinate, implement, and oversee the activity program; work with departments to schedule entertainment and classes.
  • Manage social programming at the day center, including communicating current events.
  • Assist in overseeing day floor activities in collaboration with your supervisor.
  • Conduct assessments for all participants on a semi-annual and ongoing basis.
  • Set achievable, measurable goals; schedule individualized recreational and social activities as part of care plans.
  • Communicate participant updates to team members; report concerns to the Interdisciplinary Team (IDT) as needed.
  • Document progress reports accurately in participants' medical records.
  • Explain activities and program offerings to participants, working closely with therapists and primary care staff to enhance their experience.
  • Build relationships and coordinate with community resources, including performances and holiday events.
  • Arrange recreational outings and community activities whenever possible.
  • Participate in Quality Assurance activities and facilitate discussions on program improvements.
  • Serve as a member of the IDT, attend staff meetings, and contribute to care planning.
  • Perform other duties and projects as assigned.

Qualifications

  • Bachelor's degree in recreational therapy or a related field, plus 2 years of experience serving the frail or elderly within the last 5 years. An equivalent combination of education and experience may qualify.
  • CPR and First-Aid certification required (must be obtained within 6 months of hire).
  • Must achieve Activity Coordinator State certification within the first 90 days of employment.
  • Bilingual in English and one of Cal Optima Health's threshold languages: Arabic, Farsi, Chinese, Korean, Russian, Spanish, or Vietnamese.

Preferred Qualifications

  • Experience in event planning or activity coordination.
  • Ability to design engaging activities for diverse groups with varying interests and abilities.

Knowledge & Skills

  • Ability to develop rapport and establish effective relationships with leadership, staff, and external contacts from diverse backgrounds.
  • Work independently, exercise sound judgment, and demonstrate strong problem-solving skills.
  • Excellent verbal and written communication skills.
  • Flexibility to work evenings and weekends for events and activities.
  • Organizational skills with a detail-oriented approach to planning and executing activities.
